Introduction: Marital infidelity is one of the most severe problems in families and investigating the reasons for the tendency to marital infidelity can have an effective role in reducing it.
Methods: This study was conducted based on the grounded theory (GT). To this end, two types of sampling techniques were used: The multistage random cluster sampling was employed to interview 28 teachers (male and female) and the purposeful sampling was used to interview 14 experts.
Results: Based on the concepts and categories derived from the coding of the interviews, the five main individual, marital, family, spiritual, and social factors were found to be the underlying factors for the tendency to marital infidelity. Besides, 53 concepts and 14 subcategories were identified. Based on the selective coding, individual factors were identified as the central core of the study.
Conclusion: According to our findings, marital infidelity is an individual, marital, family, spiritual and social problem. Therefore, therapists are advised to consider personal, spiritual, and social causes in addition to examining family and marital factors in dealing with infidelity.

Introduction: The hospital environment is recognized as one of the most stressful workplaces. The impacts of the hospital environment on the health-related quality of life of nurses necessitate the consideration of their viewpoints in the field of design and maintenance of the hospital environment. Accordingly, this study aimed to identify environmental factors affecting the promotion of nurses' health-related quality of life.
Methods: In this study, 10 nurses working in a selected hospital were chosen through purposeful sampling and participated in the narrative analysis. The required data were collected through narratives from semi-structured interviews with participants. The content of all narratives was evaluated by the thematic analysis.
Results: The environmental factors affecting the nurses' health-related quality of life were classified into four main themes: physical environment”, “functional support of the environment”, “social support of the environment”, and “psychological support of the environment. Besides, the themes related to the physical environment were classified under space configurations and air quality. Furthermore, functional support was placed under ergonomics. Communications and territory were found as subsets of the theme of social support, while interior design, tranquility, light quality, air quality, and the sense of control were classified under the main theme of psychological support. The frequency of themes repeated in the narratives indicated that psychological support and the physical environment have the highest impact on the health-related quality of life of nurses.
Conclusion: According to the findings of the study, policymakers, managers, and designers are recommended to try to understand nurses' perceptions of positive workplace components, and to introduce necessary environmental actions and interventions to promote the health-related quality of life of nursing staffs.

Introduction: The patient safety is a subset of organizational culture and is defined as a set of individual and organizational priorities, values, attitudes and behaviors which look for minimizing errors and damages arising from the process of patient treatment. This study seeks to describe the experiences of patients and their companions about the patient safety organizational culture and medical errors management.
Methods: This qualitative study was conducted in the health and medical education centers affiliated with Isfahan University of Medical Sciences in 2017. A total of 15 patients and companions receiving health services in medical education centers were selected using purposive sampling and in-depth interviews were conducted with the participants. Conventional qualitative content analysis was used to analyze the data.
Results: 186 initial codes, 23 sub-subcategories and 6 main themes were extracted from the data which are as follows: culture of errors acceptance vs. non-acceptance, culture of disclosing vs. hiding errors, psychological and physical consequences and financial burden of medical errors, learning from errors, the culture of patient participation and training, developing the culture of safety and all-inclusive quality improvement.
Conclusion: The results of the present study indicate a vast array of culture of non-acceptance vs. acceptance of errors and disclosing vs. hiding errors. Thus, it is imperative for the organization's senior managers to make corrective interventions so as to maintain and promote the culture of learning from errors and patient education and participation in the process of their treatment and ultimately the culture of safety and all-inclusive quality improvement.

Methods: This study was performed qualitatively, using phenomenological methods. Participates were selected from the staff and managers of Arak city governmental agencies and organizations. 42 participants were selected by purposeful sampling. The method of data collection was semi-structured interview which was recorded and rewritten with the consent of the participants and was analyzed by colazzi method.
Results: Data analysis resulted in the extraction of six main categories and several subcategories. The main categories included lack of organizational justice, change of intrinsic values of work, weakness of personality system, Weakness motivational system, Weakness socialization and poor growth of spirit of intellect.
Conclusion: The present study, by describing the weakness of the culture of work, provided a new understanding of individuals’ experiences of this phenomenon. It also links the origins of such an event to the individual factors in addition to structural factors, which are mutually interconnected.
Therefore, it is suggested that in order to change the culture of work and to achieve an excellent and progressive culture of work, at the same time, it is necessary to modify both middle-level and large-scale community structures and mechanisms, as well as to reinforce, educate, and change its individual and personal contexts.
